Menceritakan tentang para alkemis yang mempelajari dan melaksanakan seni transmutasi alkimia - untuk memanipulasi objek dan mengubah satu objek ke dalam bentuk lainnya. Namun walaupun begitu, mereka tetap dibatasi oleh hukum dasar alkimia: untuk mendapatkan sesuatu, maka Anda harus mengorbankan sesuatu dengan sesuatu yang setimpal.
Karakter utama dalam anime ini adalah seorang alkemis terkenal yang bernama Edward Elric - juga dikenal sebagai Fullmetal Alchemist - yang hampir kehilangan adiknya, Alphonse, dalam sebuah kecelakaan. Edward berhasil menyelamatkan jiwa adiknya, namun Edward harus membayar dengan harga yang setimpal, Edward terpaksa mengorbankan sebagian anggota tubuhnya.
